Individual access planning
Start with the person, not the diagnosis.
These profiles surface questions that may matter. They do not predict needs from a condition and do not replace clinical or occupational advice.
Mobility and positioning
- Which movements or positions are comfortable and sustainable?
- Can every release and exit be reached independently or with agreed support?
- What happens if strength, balance or spasm changes suddenly?
Fatigue and fluctuating energy
- What are early signs that energy is falling?
- Can the plan be shortened or stopped without disappointment or pressure?
- What recovery time and practical support may be needed?
Chronic pain and sensation
- How will ordinary symptoms be distinguished from a warning sign?
- Which pressure, texture, temperature or duration should be avoided?
- Does medication change awareness, bleeding or recovery?
Sensory processing
- Which light, sound, smell, touch or uncertainty may overload?
- What predictable sequence or advance information would help?
- How will a need for silence, space or immediate change be communicated?
Communication and neurodivergence
- Which words, formats or response time support genuine understanding?
- Is a non-speaking pause or stop method needed?
- How will literal meaning, uncertainty and changed decisions be checked?
Hearing and vision
- Can essential information be received without relying on one sense?
- Are lighting, contrast, captions, tactile markers or interpreters relevant?
- Will communication remain accessible during the entire activity?
Privacy, carers and support
- Who needs to know what, and who does not?
- What is the role and boundary of any assistant, carer or supporter?
- How will autonomy, confidentiality and safeguarding coexist?
